A Partnership with Covenant Health Network

Covenant Health Network (CHN) is one of the nation’s preeminent, non-profit, post-acute care alliances dedicated to providing resident-centered models of care and member services that balance high quality with efficiency to a growing membership of senior care providers. CHN is a non-profit 501(c)(3), which was formed in 1997. It is a strategic alliance of 111 non-profit communities that have made a commitment to improve the quality of care and quality of life for seniors in congregate settings through joint quality improvement processes while reducing the costs of operation. Covenant is headquartered in Phoenix, Arizona, with about half its member organizations in Arizona. CHN member communities serve thousands of seniors daily in a wide variety of placement settings in Arizona, Colorado, Mississippi, New Mexico, and Pennsylvania.

Symbria Rehab HVA

A Partnership with HVA Senior Living Alliance

HVA Senior Living Alliance is an alliance of regional, not-for-profit, senior living communities in western Pennsylvania. Committed to offering the best possible quality of life for residents, HVA’s members are mission-driven, and through the alliance, they realize the greatest efficiency and economy in improving and sustaining the quality of each community’s services. Together they pursue staff education and training, negotiate low-cost contracts for supplies, obtain grants to help defray the costs of training and operations, and share best practices for collective improvement. As not-for-profit organizations, residents are at the center of the HVA Senior Living Alliance value system. Their quality of life comes first.

A Partnership with CALTC Health Ventures

CALTC Health Ventures is a statewide network of premier not-for-profit providers offering a full continuum of post-acute-care services including long-term care, sub-acute care, rehabilitation, and home care and retirement services. CALTC supports its membership through a variety of value-added management and clinical services, such as business development, quality assurance, patient satisfaction, and managed care contracting.
