Symbria SAFE PSO will help improve resident care

At Symbria Rx Services, we are committed to providing the highest standard of care for your residents. That's why we are pleased to announce that our pharmacy has joined the Symbria SAFE Patient Safety Organization (PSO).

The PSO model, which was created as part of the Patient Safety and Quality Improvement Act of 2005, encourages licensed pharmacy staff to confidentially share quality, dispensing error, and medication error information to improve the safety of health care delivery without the risk of litigation. This protection allows our pharmacy to perform root-cause analyses of medication errors in a safe environment that protects employees, Symbria Rx Services, and our clients from the possibility of this information being subpoenaed.

Performing a root-cause analysis has always been a top priority for our pharmacists in the rare case of a medication error; however, we had to be selective about the information we shared in the past to minimize our risk. The PSQIA now provides a safe environment for our pharmacists to discuss how an error may have occurred and what action is required in the future to ensure patient safety and improve the quality of medicine dispensing.

To receive the protection from litigation that the PSO provides, we are also prohibited from disclosing patient safety information to anyone outside of the PSO. If your community would like understand what we are doing to prevent a medication error in the future, we recommend that employees from your community join the Symbria SAFE PSO.

"Our pharmacy can now collect information about medication errors, determine why the errors occurred, and recommend aggregated best practices that our client communities can use to help improve resident safety."
